We’re excited to see you! We understand that visiting an obstetrician-gynecologist can be both exciting and nerve-wracking, especially if it's your first time or you're preparing for an important appointment. Here’s a short guide of what to expect.

What to Bring
If you are new to our practice, please download and fill out the registration forms below. Then, fax, mail, or bring the completed paperwork to your appointment. If you are established with us, you can use these forms to let us know of any changes in your health history or insurance information.
If you are unable to complete the registration process prior to your visit, please arrive 15-20 minutes before your scheduled appointment time as check-in time is dependent upon completion of the required forms.

1180 Beacon Street is situated at the northwest corner of the intersection of Beacon Street and Kent/Powell Street near Massachusetts’ renowned Longwood Medical Area. It is located across the street from the Kent Street stop on the Cleveland Circle (C) Branch of the MBTA’s Green Line.

Understanding your Visit
For Obstetrics Appointments
At the beginning of your pregnancy, we will review the schedule of your visits. We will remind you of upcoming tests and relevant paperwork due at the next visit.
For Gynecology Appointments
For an annual exam, it is helpful to know the approximate date and result of your most recent screening tests (pap smear, mammogram, colonoscopy, bone density scan, etc.).
For a problem visit, please send ahead or bring any relevant lab results or imaging study reports.
For Telehealth Visits
We use Doximity for our telehealth visits. At the time of the visit, we will text your phone with a link. Please choose a quiet space with good internet wifi or cellular signal, and enable your microphone and camera.
For Procedures
For most procedures, it is helpful to take a pain reliever like acetaminophen or ibuprofen about one hour before your office procedure. For intrauterine devices and endometrial biopsies, please inquire about a prescription medication to soften the cervix.
